Background and Rationale

Public Health’s Environmental Health and Clinical Services divisions operates several programs that rely on license fees, clinical service fees, vaccination/medication fees, and other fees to cover regulatory expenditures as well as expenditures for services performed by the clinic. The Ramsey County Fee Schedule in the 2026-27 budget was approved by the Ramsey County board in December 2025, but did not include the updates and proposed changes submitted by Environmental Health and Clinical Services. The proposed changes to be approved are attached.
Since 1990, Environmental Health programs for licensing and related regulatory activities have been license-fee funded. (Resolution 90-634). The fees generated for regulatory activities do not receive any tax, levy or state funds. Portions of the programs that are not related to licensing and inspection are funded with County Environmental Charge funds.

• Lodging Establishments
• Manufactured Home Parks
• Public Swimming Pools Solid & Hazardous Waste Compliance
• Hazardous Waste Generators
• Hazardous Waste Facilities
• Solid Waste Facilities Solid Waste Operations
• Very Small Quantity Generator collection
Clinical Services fees have been an integral part of providing services to the community for our clinic that allow residents to receive services for little to no-cost. These fees are established by evaluating private market rate through insurance as well as Public Managed Assisted Program. These fees include the cost of medication, services and supplies.

Racial Equity Impact
The racial equity impact of departments, programs and services is included in the 2025-2026 Biennial Budget documents and was previously discussed during the budget hearings in September of 2025. There is no known racial equity impact for Environmental Health. Clinical Services for fees are evaluated to ensure that there are no barriers to accessing care.
Community Participation Level and Impact
Each service team held budget engagement sessions with staff from their respective service teams. In addition to staff engagement sessions, Ramsey County held community budget townhalls to share information on the county’s budget process and county priorities. The sessions included questions and answers as well as community feedback segments.

Fiscal Impact
The corrected fee schedule should collect sufficient funds to meet revenue expectations in the approved 2026-2027 budget. The magnitude of the correction cannot be estimated because total fee revenue can vary considerably from year to year, based on several factors. Those factors include the number of establishments that open and close, waste volumes generated, etc. There have been years when, despite a change in limited fees, the revenue target was not reached and other years when it was exceeded.
Clinical Service Fees are collected to cover the cost of care, in the approved 2026-2027 budget.
Last Previous Action
On December 16, 2025, the County Board adopted the current fee structure when it approved the 2026-2027 Operating Budget (Resolution B2025-246).
