2. Authorize the Chair and Chief Clerk to execute the agreement.

Background and Rationale

In 2023, Dakota County Public Health reached out to Saint Paul - Ramsey County Public Health for a first time ever Joint Powers Agreement to provide tuberculosis control services to Dakota County residents. Saint Paul - Ramsey County Public Health is requesting to continue the Joint Powers Agreement to provide tuberculosis (TB) detection, prevention and treatment services to clients residing in Dakota County, who have been referred by Dakota County Public Health. These tuberculosis control services include TB skin testing; chest x-rays; laboratory services; physician evaluation, consultation, diagnosis, and treatment; medication dispensing and monitoring; follow-up services; and additional diagnostic services. Clients with active or latent TB will be referred to Saint Paul - Ramsey County Public Health for services and will work with Dakota County Public Health to ensure that the continuation of services will occur until clients are successfully treated.
Ramsey County bills third party payers for all services provided. For clients who are uninsured, Dakota County will reimburse all services rendered at the Ramsey County’s board approved rate during the term of the contract. Other reimbursements include $1,700 a year for interpreter costs and a one-time fee for case management services for each TB client referred who does not have insurance coverage. Ramsey County does not receive any other administrative payment for services rendered.

Racial Equity Impact
TB disproportionately impacts people who are within congregate settings, including correctional facilities, drug treatment centers, nursing homes and homeless shelters. Foreign-born persons, including children are highly impacted by TB. Control and management of TB is critical to prevent the further spread of TB within communities most impacted.
Community Participation Level and Impact
Residents of Dakota County who are referred to Saint Paul - Ramsey County Public Health will be informed of eligibility to receive TB control services.

Fiscal Impact
The $70,000 is included in the approved 2026-2027 Public Health budget.
Last Previous Action
On December 19, 2023, the Ramsey County Board of Commissioners approved the Joint Powers Agreement with Dakota County for Tuberculosis Control Services for the period of January 1, 2024 through December 21, 2025 (Resolution B2023-254).
